Middle School & Teen
- Immunization Records (non-DoD card holder 6-12th grades only- $18 registration fee)
- DA 7625-1 Form
- DD 2652 Form (for summer camp only)

- Special Needs Declaration. If your child has a special need (i.e. asthma, learning disability, allergy, ADHD, etc) a screening by the Special Needs Accomodation Process Team will be required before starting.
